The neighbourhood health framework sets out minimum goals over the next three years to: improve health outcomes; improve access to GPs; improve experience of planned care; better urgent and emergency care performance; and improve patient and staff satisfaction. A selection of the main targets under the five goals are listed below.
Goal 1: improve health outcomes
- reduce non-elective admissions and bed days of one day or over by 10% for people with mid to severe frailty
- increase the number of people identified as approaching end of life by 10%
- reduce non-elective admissions and bed days of one day or over for people in the end of the life by 10%
- improvement of at least 10% in evidence-based clinical outcomes for CVD, diabetes, COPD, mental health conditions and dementia
- increase the percentage of patients with diabetes who receive all 8 elements of the diabetes care process bundle in the preceding 12 months by 10%
- reduce acute outpatient appointments for children under the age of 16 by 10%.
Goal 2: improve access to general practice
- aim to see 90% of clinically urgent patients on the same day by March 2027
Goal 3: improve experience of planned care
- reduce variation in referrals to outpatient services across system through single point of access to at least 25% by March 2027 for at least 10 high volume specialties, supporting overall RTT trajectories of 70% by March 2027 and 92% by March 2029
- contribute to an overall reduction in secondary care follow-up appointments by at least 10% by March 2027
Goal 4: better urgent and emergency care performance
- contribute to an increase in type 1 emergency department (ED or A&E) admitted and non-admitted performance, supporting overall 4-hour trajectories of 85%, with an interim trajectory of 82% by March 2027
Goal 5: improve patient and staff satisfaction
- By 2027, 95% of people with complex needs to have an agreed care plan.
