Trust 'missed vital warning sign' patient had developed lung cancer

South Tyneside and Sunderland NHS Foundation Trust has apologised after a failure to diagnose lung cancer earlier contributed to a patient’s death.

Anne Lorraine Dyson died at St Benedict's hospice on 24 February 2025 having been diagnosed with metastatic lung cancer on 20 November 2024.

A prevention of future deaths report found Dyson had been under investigation for lung disease since September 2021 with scans showing an increased growth from October 2023.

Coroner David Place found an incorrect interpretation of a CT scan on 25 March 2024 led to a significant delay of many months in diagnosing a malignancy which had progressed to be terminal and could no longer be  successfully treated. 

Ben Hall, clinical director of diagnostic imaging for the trust, said: ‘We offer Anne's family our heartfelt condolences following their loss. We understand this has been a very difficult time for her loved ones.

‘We know from the review we have carried out that we missed a vital warning sign she had developed lung cancer. This should not have happened and we apologise for this.

‘We have thoroughly investigated this and shared learnings with our wider radiology team with the aim this will not happen again.'
