Somerset ICB selects digital continuing healthcare platform

NHS Somerset has selected IEG4 to implement a digital continuing healthcare platform, aimed at streamlining its All Age Continuing Care process.

The solution, developed by IEG4, promises to enhance patient experience, improve clinical efficiency, and support better health outcomes. Previously, NHS Somerset faced challenges with its platform that demanded extensive manual input, particularly with staff entering data into property fields.

The new system will meet NHS Somerset's requirement for a platform that can be accessed by both staff and patients and referral forms that can be digitised and automated. It will also include a portal where users can access immediate updates on progress, while accessing administrative and payment tools where needed, giving full end-to-end visibility.

The IEG4 platform also maps data fields to meet the required fields in the patient level data set (PLDS), which enables an ICB to generate an extract of the data required for the PLDS.

Lynette Emsley, associate director of continuing healthcare and care sector services, said: ‘Putting in place the new digital platform for continuing healthcare in Somerset, will make it easier for individuals and their family members/representatives to be involved with the CHC assessment process and for patients and staff to readily access the information they need directly.'
